Another Child Served Alcohol at a Resturant?

What is going on? Another child was served an alcoholic drink at a restaurant!  This time at a Chili's outside of Chicago. A woman said she ordered a chocolate milkshake for her 4 year old daughter. The toddler  took four sips of it before she said it tasted funny. Seems that the waitress had served the child a "Mudslide"! The child's  mother immediately called the cops and took her wasted little girl to the emergency room.

Please help me understand what is going on! I know it's been over 20 years since I worked in a restaurant, but as a waitress,  you had to go to the bar area to order alcoholic drinks for a customer. All other drinks like cokes, tea, coffee and yes milkshakes were in another area.  It would have been VERY, VERY difficult to mix these drinks up. I just don't get it! Luckily, this hasn't happened in our area and hopefully never will.
